To ensure the safety and compatibility of our program with a participant, we ask that parents/guardians complete the following documents:
We have included two versions of the Participant Profile Form for the participant to complete depending on their literacy level.

Once your forms have been received, you will be contacted for an intake meeting to outline the specifics of our programming as well as answer your questions.
Please note: Connection of Friends reserves the right to deny participation to anyone in the program that we solely determine will be unable to participate.
Once all the forms are received and the reference checks are completed, our Program Director will call and schedule a day and time for parents and/or guardians and prospective participants to attend an in-take at Connection of Friends. Parents and/or guardians will meet with the Executive Director while the prospective participant meets with the Program Director and Program Leader for a tour and a series of group activities. A schedule will be provided prior to the in-take outlining the specific activities planned.
After the in-take meeting is complete, our Program Director will be in contact to answer any further questions and to discuss future registration.
